This Home Health Physical Therapist opportunity is based in the welcoming community of Franklinton, North Carolina and the surrounding areas. In this role, you will conduct comprehensive initial evaluations in the home, assess functional mobility, strength, balance, and safety, and develop individualized, evidence-based treatment plans that align with patient goals. Typical responsibilities include therapeutic exercise, gait and transfer training, fall risk assessments, home safety recommendations, and instruction in assistive device use. You will provide detailed education to patients and caregivers on home exercise programs and strategies to optimize independence and quality of life. Each day generally includes a series of scheduled home visits, travel between residences, documentation of your findings and interventions in an electronic medical record, and coordination of care with an interdisciplinary team. You will work closely with nurses, occupational therapists, speech therapists, and other clinicians to ensure a cohesive plan of care, timely communication, and strong patient outcomes. Caseloads typically focus on adult and geriatric patients, with visit volumes structured to support thorough, high-quality care and accurate documentation. The environment supports professional growth and the development of clinical expertise in home-based rehabilitation. You will gain valuable experience working across a range of diagnoses, including orthopedic post-operative cases, neurologic conditions, cardiopulmonary limitations, and generalized deconditioning.
