This Home Health Physical Therapist opportunity is based in the welcoming community of Franklinton, North Carolina and the surrounding areas. You will provide detailed education to patients and caregivers on home exercise programs and strategies to optimize independence and quality of life. Each day generally includes a series of scheduled home visits, travel between residences, documentation of your findings and interventions in an electronic medical record, and coordination of care with an interdisciplinary team. You will work closely with nurses, occupational therapists, speech therapists, and other clinicians to ensure a cohesive plan of care, timely communication, and strong patient outcomes. Caseloads typically focus on adult and geriatric patients, with visit volumes structured to support thorough, high-quality care and accurate documentation. The environment supports professional growth and the development of clinical expertise in home-based rehabilitation. You will gain valuable experience working across a range of diagnoses, including orthopedic post-operative cases, neurologic conditions, cardiopulmonary limitations, and generalized deconditioning.
