Home Health Physical Therapist jobs in Rapid City, SD let you provide care to medically compromised patients with diagnoses like CHF, COPD, DM, cardiac, ortho, neuro, dementia, and general weakness. 5x8 schedule You will evaluate, plan, and deliver therapy to adults and geriatrics in their homes, using EPIC for charting.
