This Home Health Physical Therapist role is based in Marble Falls, Texas, a vibrant community in the picturesque Texas Hill Country. As a Home Health Physical Therapist, you will provide one-on-one, patient-centered care in the comfort of patients’ homes across Marble Falls and surrounding areas. You will perform comprehensive evaluations, develop individualized treatment plans, and deliver skilled interventions aimed at improving mobility, independence, and quality of life. Typical patient populations include adults and older adults recovering from orthopedic procedures, managing chronic conditions, or experiencing functional decline and increased fall risk. A typical day may include a series of scheduled home visits, completion of clinical assessments, coordination of care with nursing and other therapy disciplines, and documentation of progress using an electronic medical record system. Responsibilities include conducting thorough physical therapy assessments, setting measurable functional goals, and implementing evidence-based interventions tailored to each patient’s environment and support system. You will provide gait and balance training, strengthening and endurance exercises, transfer training, home safety evaluations, and education on the proper use of assistive devices and durable medical equipment. You will also play a key role in fall prevention and rehabilitation following hospitalization, surgery, or acute illness, working closely with the broader home health team to ensure continuity and quality of care. This position offers a dynamic schedule that may include full-time day shifts with flexibility based on patient needs and caseload. You will typically manage your own daily route and visit schedule, allowing for autonomy and the ability to prioritize patient care while maintaining efficient documentation practices. Caseload size and visit frequency are designed to support safe, high-quality care and meaningful patient interaction rather than rushed encounters. The local home health environment is well-suited to practitioners who value strong relationships with patients, families, and other clinicians.
