This Home Health Physical Therapist Assistant position offers the opportunity to practice in one of the Pacific Northwest’s most desirable communities. Kirkland, Washington is a lakeside city along the shores of Lake Washington, known for its walkable waterfront, boutique shops, art galleries, and excellent dining. In this role, you will provide home-based physical therapy assistant services to patients in Kirkland and nearby areas. You will work closely under the direction and supervision of a licensed Physical Therapist, delivering evidence-based interventions that help patients regain independence and function in the comfort of their own homes. A typical day may include traveling between patient residences, completing initial functional assessments as directed, implementing established therapy plans of care, progressing therapeutic exercises, assisting with gait training and mobility practice, and providing thorough patient and caregiver education on safety, home exercise programs, and strategies to reduce fall risk. You will manage a structured but dynamic caseload, generally focusing on adult and geriatric patients with conditions such as post-surgical orthopedic issues, stroke, generalized weakness, cardiopulmonary diagnoses, and other chronic illnesses impacting mobility and function. Visit frequency and patient ratios are designed to support safe, high-quality care and allow time for meaningful one-on-one interaction. You will perform in-home safety evaluations, help identify environmental barriers, and recommend modifications or adaptive strategies to support each patient’s rehabilitation goals. The position offers a schedule that is typically daytime and weekday-focused, with some flexibility based on patient needs and agency requirements. Home health work provides a high degree of autonomy: you will organize your daily route, coordinate visit times, and collaborate with nursing, occupational therapy, medical social work, and other disciplines to ensure cohesive care. Electronic medical record systems are used for documentation, care coordination, and communication, with training and ongoing support provided.
