This Home Health Physical Therapist opportunity offers the chance to practice in beautiful Volusia County, Florida, a region known for its vibrant coastal lifestyle and welcoming communities. In this role, you will provide one-on-one rehabilitative care to patients in their homes across Volusia County. You will manage a diverse caseload that may include post-surgical orthopedic patients, individuals recovering from stroke or neurologic conditions, chronic disease management cases, balance and fall risk patients, and those requiring strengthening, endurance, and functional mobility training. This position offers a high level of autonomy and the opportunity to form meaningful therapeutic relationships while helping patients remain safely in their homes. Typical responsibilities include performing comprehensive evaluations, developing individualized plans of care, implementing evidence-based interventions, educating patients and caregivers on home exercise programs and safety strategies, and collaborating with an interdisciplinary team. You will coordinate closely with referring providers, nursing, occupational therapy, and other disciplines to support optimal outcomes and readmission prevention. Daily documentation is completed via electronic systems, with an emphasis on accuracy and timeliness to support quality and regulatory compliance. The typical day involves traveling within an assigned territory in Volusia County to complete scheduled patient visits, prioritizing efficiency while maintaining strong clinical care. You may see a range of visit types, including start-of-care assessments, routine follow-ups, reassessments, and discharge visits. Visit frequency and caseload are structured to allow focused, quality time with each patient while supporting reasonable daily productivity expectations. You will have the opportunity to tailor interventions to the home environment, address real-world barriers, and enhance safety and independence where patients live and function. This position is ideal for a Physical Therapist who enjoys independence, community-based care, and the ability to practice at the top of their license. Licenses: PT-FL License and HH experience are required.
