This Home Health Physical Therapist position is based in the Elizabethtown, Kentucky area, a friendly and vibrant community in central Kentucky. You will work with a variety of adult and geriatric patients, including those recovering from surgery, managing chronic conditions, addressing balance and fall risk, or needing support to remain safely at home. Therapy interventions may include strength and conditioning programs, gait and transfer training, home safety assessments, pain management strategies within your scope of practice, and caregiver education. You will regularly reassess progress, adjust plans of care, and coordinate closely with an interdisciplinary team to ensure high-quality, patient-centered care. Communication with nurses, occupational and speech therapists, social workers, and physicians is a key part of the role, helping to coordinate services and achieve optimal outcomes. You can expect to manage a home health caseload with visits scheduled throughout the day, typically during daytime hours, with potential flexibility depending on organizational needs. Documentation will be completed using an electronic medical record system, and you will be responsible for maintaining accurate, timely records in accordance with regulatory and professional standards. While exact patient ratios and daily visit counts may vary based on acuity and service area needs, the goal is to support safe, efficient care that allows you to spend meaningful time with each patient.
