This Speech-Language Pathologist position is based in a skilled nursing and rehabilitation setting in Grants Pass, Oregon, a welcoming small city known for its beautiful Rogue River, vibrant outdoor lifestyle, and strong sense of community. In this role, you will join a collaborative interdisciplinary team dedicated to providing high-quality, patient-centered care for adult and geriatric residents requiring skilled nursing, post-acute rehabilitation, and long-term support. You will work closely with physical and occupational therapists, nursing staff, physicians, and social services to design and implement comprehensive care plans that address the whole person. Typical responsibilities include conducting thorough speech, language, cognitive-communication, and swallowing evaluations; developing individualized treatment plans; delivering evidence-based therapies; and providing education to patients, families, and staff. You can expect to manage a caseload that may include diagnoses such as stroke, brain injury, dementia, Parkinson’s disease and other neurological conditions, as well as medically complex post-surgical and cardiopulmonary patients. Dysphagia assessment and treatment are key aspects of this role, including bedside swallow evaluations, development of safe diet and liquid recommendations, and collaboration with nursing and dietary services to ensure safe, enjoyable mealtimes. You will also be involved in cognitive-communication treatment to support safety, memory, problem-solving, and functional communication needed for daily living and discharge planning. Your typical day in this skilled setting includes morning team check-ins, evaluations of new admissions, scheduled treatment sessions, documentation in the EMR, participation in care conferences, and regular communication with families. Productivity expectations are balanced with an emphasis on quality outcomes, resident dignity, and functional progress. Therapy is delivered primarily one-on-one, with occasional group or co-treatment sessions when clinically appropriate. Caseload and patient ratios are designed to support meaningful time with each resident while maintaining regulatory and clinical standards. The therapy spaces are designed to support functional rehabilitation, with dedicated treatment areas and access to adaptive equipment to simulate real-world tasks. You will have the opportunity to contribute to program development, such as expanding cognitive rehab, communication groups, or specialized dysphagia initiatives tailored to the needs of the resident population.
