Description
Job Summary:
This position is for a Physical Therapist (PT) within the Home Health specialty, located in Oak Brook, Illinois. The role involves providing in-home therapy services to adult and geriatric patients, with a focus on assisting with activities of daily living and adaptive equipment. Candidates are required to bring their license on the first day of placement.
Job Details:
- Location: Oak Brook, Illinois
- Schedule: 8:00 AM - 5:00 PM, M-F, minimum 40 hours per week
- Required Qualifications: Active Illinois Physical Therapist (PT) license, BLS (American Heart Association) certification, valid Driver's License and auto insurance, at least 2 years of Physical Therapist Home Health experience with adult and geriatric patients, experience assisting with Activities of Daily Living (ADLs) and adaptive equipment, ability to collaborate with other specialties including Registered Nurses, Occupational Therapists, Speech Therapists, etc.
- Preferred Qualifications: Recent Home Health experience within the last year, experience working with adult/geriatric patient populations.
- Additional Information: Candidate must have reliable transportation to visit patients, be able to assist with ADLs and adaptive equipment, and collaborate with a multidisciplinary team. Orientation to EMR via EPIC will be provided. Position requires travel within the specified coverage area, which includes various zip codes in the central Chicago metropolitan area. No weekend, holiday, or call responsibilities are scheduled.
- Support on the Unit: Charge nurse, nursing supervisor, manager, and team leads are available for support.
- Technology/Equipment: EMR system: EPIC; Monitors: N/A; IV pumps: Sapphire; Medication dispensing: Not applicable.
- Float: Float rotation may be required within the specified coverage area.
- Attire: Any solid color scrubs or business casual attire are acceptable; jeans and open-toe shoes are not permitted.
- Transportation: The traveler must provide their own transportation to all patient visits.
