Description
Job Summary: Physical Therapist (Outpatient) in Bristol, Pennsylvania. The outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies individualized treatment plans aligned with patient needs, goals, and physician direction. The role includes completing clinical documentation, ensuring care is provided under signed and valid physician orders, and contributing to professional development and performance improvement efforts. Candidates are required to bring their license on the first day.
Job Details:
- Schedule: Days shift, 8 hours per shift, with a minimum of 40 hours per week.
- Start Date: 07/06/2026
- Required Qualifications: Degree in Physical Therapy from an Accredited School; 1+ Year of Outpatient Clinical Experience; CPR Certification; active/applicable license for the State where work will be performed.
- Preferred Qualifications: Has this employee successfully completed an assignment with your agency previously?
- Responsibilities:
- Complete comprehensive evaluations of assigned patients using clinical reasoning to develop treatment plans and discharge-focused goals.
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing and required functional documentation.
- Ensure therapy services are provided under signed, valid physician orders and within applicable insurance approval processes as needed for clinical care.
- Participate in student programming as assigned, including providing supervision, supporting learning objectives, and completing student evaluations.
- Create and maintain an individualized professional development plan; pursue ongoing professional growth through continuing education, self-study, and professional activities.
- Adhere to the discipline-specific State Practice Act and other applicable regulatory requirements; maintain licensure in good standing.
- Perform in an ethical manner in accordance with discipline-specific professional codes of conduct.
- Support organizational performance improvement by following action plans and providing feedback through appropriate channels.
