Schedule: 8-hour days, minimum 40 hours per week
Required Qualifications: Valid Physical Therapist (PT) license; BCLS certification; 1 year of skilled home health experience; experience with acute care, outpatient clinic, rehabilitation hospital, and skilled nursing facility (SNF); knowledge of various diagnoses and treatments listed; reliable transportation.
Preferred Qualifications: Home health experience, experience with Oasis, EPIC charting system, and familiarity with patient education and therapy equipment.
Additional Information: The position requires the therapist to work within a generally 60-mile radius from the assigned address, with occasional travel up to 90 minutes daily. The role involves minimal patient visit ratios and charting using EPIC and Rover/Oasis systems. Travelers are expected to float between facilities as needed.
Certifications: BCLS (Basic Life Support)
Shift Time: Days, 8 hours per shift, with a minimum of 40 hours per week
Other Requirements: References (2) covering 1 year (6 months for CNAs) from within the last 3 years; ability to meet point expectations; confirmed phone number.
