Job Summary:
Home Health Physical Therapist (PT) in Medford, Oregon. This position involves providing skilled physical therapy services to patients in a home health setting, serving diverse diagnoses including post-surgical, neurological, and musculoskeletal conditions. Candidates are required to bring their license on the first day of assignment.
Job Details:
- Specialty: Physical Therapist Home Health
- Shift: Days, 8 hours per day
- Schedule: Please note, the facility cannot accommodate 4x10s or 3x12s shifts.
- Required Qualifications: BCLS certification; 1 year of skilled home health experience; valid license; reliable transportation; experience with electronic charting systems such as EPIC/Oasis.
- Preferred Qualifications: Home health experience preferred.
- Float: The traveler is expected to float between all facilities within a given state; refusal to float may result in termination or forfeiture of guaranteed hours.
- Additional Information:
Traveler is expected to have their own transportation. The system groups clients efficiently, minimizing drive and windshield time, with occasional radii up to 50+ miles. The facility requires primary series COVID vaccination plus one booster. Oasis charting system experience is mandatory. - Certifications: BCLS
