Job Summary: Physical Therapist, Outpatient Avondale, AZ. The organization is seeking a Physical Therapist to provide outpatient care for adults and adolescents. Responsibilities include case management and conducting gross motor assessments while delivering treatment for a range of orthopedic and musculoskeletal conditions. Candidates are required to bring their license on the first day.

Job Details:

• Schedule: Days shift; 8 shift hours; minimum 40.0 hours per week (manager prefers 5x8 but is open to 4x10s).
• Start Date: 07/06/2026
• Responsibilities: Case management, gross motor assessment, and outpatient physical therapy services.
• Required Qualifications: Physical Therapist licensure; outpatient PT experience; 1+ years of experience; experience working with adults and adolescents; familiarity with Electronic Medical Records (EMR) systems (NextGen).
• Preferred Qualifications: Outpatient experience.
• Additional Information: Treatment areas may include Achilles tendonitis, ACL tear, post-operative back pain, calf strain, disc herniation, fractures/stress fractures, generalized low back pain, headache, meniscus tear, osteoarthritis, osteoporosis, plantar fasciitis, postural dysfunction, rotator cuff non-surgical intervention, SI joint dysfunction, rotator cuff repair post-operative care, stenosis, thoracic outlet syndrome, and total joint replacement. Typical workload is 5055 visits/week. Dress code is business casual.
