Job Summary
Physical Therapist, Physical Therapist SNF Paso Robles, CA. The physical therapist implements patient rehabilitation programs to improve patient mobility, function, and strength in an outpatient/rehabilitation setting. This role requires evaluating and treating patients using a thorough clinical process, maintaining accurate documentation in an Electronic Medical Record, and guiding patient care through collaboration with the care team. Candidates are required to bring their license on the first day.

Job Details

• Schedule: Days shift; 8 hours per shift, 40.0 hours per week, with a total schedule of 5 days per week (8 hours/day, 5 days/week).
• Required Qualifications: Degree in Physical Therapy; must hold active license or plan to test in the near future to hold license as a Physical Therapist in the state in which you intend to work.
• Preferred Qualifications: Continuing Education courses.
• Responsibilities:
  • Evaluate and treat patients for physical therapy and rehabilitation programs.
  • Utilize Electronic Medical Record to complete evaluations, reports, and discharge summaries for patient files.
  • Guide and monitor patient care in all aspects of treatment and follow-up.
  • Supervise Physical Therapy Assistants and Aides regarding all aspects of patient care.
  • Communicate with physicians and other persons to ensure quality patient care.
  • Coordinate with coworkers and other offices regarding scheduling and patient follow-up.
  • Maintain accurate patient chart documentation and complete appropriate follow-up related to prescriptions, test results, and authorizations.
  • Ensure patients are comfortable and respond to questions as necessary.
  • Make independent decisions regarding patient care.
  • Participate in business marketing and referral activities when necessary.
  • Follow Health Insurance Portability and Accountability Act (HIPAA) policies and procedures.
  • Comply with Occupational Safety and Health Administration (OSHA), federal, and state regulations.
  • Be familiar with Physical Therapy billing policy and procedure and be proficient in Diagnosis (DX) and International Classification of Diseases, 10th Revision (ICD-10) coding for the billing of services.
  • Provide physical mobility and support as required, including walking, sitting, standing, bending, lifting, and assisting with transfers and patient safety needs.
