Job Summary: Sun City, AZ Physical Therapist (Outpatient)
The organization is seeking an Outpatient Physical Therapist with a focus on Orthopedics and Geriatrics. In this role, the Physical Therapist evaluates assigned patients, develops and modifies individualized treatment plans, and supports safe, effective return to community activities. Candidates are required to bring their license on the first day.
Job Details:
- Schedule: Days shift; 8 hours per shift; minimum 40 hours per week.
- Required Qualifications: Degree in Physical Therapy from an accredited school; CPR Certification; active/applicable license for the State where work will be performed; and 1+ year of outpatient clinical experience.
- Preferred Qualifications: Prior successful completion of an assignment with the organizations agency; and prior successful completion of an assignment with the organization.
- Responsibilities: Evaluate assigned patients and use clinical reasoning to develop treatment plans and goals focused on progressing patients toward discharge; complete medical record documentation, including goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry using approved formats; ensure care is provided under signed, valid physician orders and in alignment with insurance approval when applicable; delegate and direct discipline-specific therapy services for assigned patients; develop functional treatment programs that support effective return to community activities; educate patients, families, students, coworkers, and self; participate in student program activities as assigned with appropriate supervision and student evaluation; maintain professional licensure in good standing and complete state-mandated continuing education requirements; adhere to discipline-specific State Practice Act and other regulatory requirements; understand and follow organizational performance improvement methodology and support related action plans.
- Additional Information: Must follow clinic guidelines outlining standards of personal appearance, attendance, and punctuality; uphold compliance expectations, including confidentiality regarding patient and organization information; demonstrate behaviors and attitudes consistent with the organizations Mission Statement and Core Values, including a strong focus on patient and employee safety.
