Job Summary
Physical Therapist (Outpatient) Surprise, AZ. The organization is seeking an Outpatient Physical Therapist to evaluate assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction. Candidates are required to bring their license on the first day.

Job Details

• Schedule: Days; 8 shift hours; minimum 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an accredited school; CPR Certification; active/applicable license for the state where work will be performed; 1+ year of outpatient clinical experience.
• Preferred Qualifications: Prior assignment with the organization; prior assignment with the organization previously.
• Develops and updates functional treatment programs to support effective return to community activities.
• Provides education to patients, families, students, coworkers, and self.
• Performs professional duties related to patient care, development of discipline-specific therapy services, and delivery of treatment programs.
• Completes comprehensive evaluations and uses clinical reasoning to establish treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
• Completes timely and accurate medical record documentation, including goal writing and related documentation in accordance with clinic policies and procedures.
• Ensures care is provided under signed, valid physician orders and as indicated and necessary for clinical care.
• Participates in student programs as assigned, providing supervision, supporting learning objectives, and completing student evaluations.
• Creates an individualized professional development plan with clinical management, pursues continuing education and professional growth, and applies learned information to support organizational objectives.
• Adheres to the discipline-specific State Practice Act and other regulatory agencies; maintains professional licensure in good standing and completes state-mandated continuing education requirements.
• Performs ethically in accordance with discipline-specific professional code of conduct.
• Supports organizational performance improvement methodology by following action plans and providing feedback through appropriate channels.
