Description
Job Summary:
Physical Therapist Outpatient in Girard, Pennsylvania, is responsible for evaluating assigned patients and developing, implementing, and modifying individualized treatment plans aligned with patient needs, goals, and physician direction. The role includes documentation, participation in student programming as assigned, and supporting patient progress toward discharge. Candidates are required to bring their license on the first day.
Job Details:
- Schedule: Days shift; 8 hours per shift; minimum 40 hours per week
- Required Qualifications: Degree in Physical Therapy from an accredited school; 1+ year of outpatient clinical experience; CPR certification; active/applicable license for the State where work will be performed
- Preferred Qualifications: Successfully completed an assignment with the organization previously; successful completion of an assignment with the organization previously
- Responsibilities:
- Complete comprehensive evaluations of assigned patients
- Use clinical reasoning to create treatment plans and goals focused on progressing patients toward discharge within an established length of stay
- Maintain clear, concise, accurate, and timely medical record documentation, including documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry in approved formats
- Ensure care is provided under signed, valid physician orders and within insurance approval, as indicated and necessary for clinical care
- Participate in student program activities as assigned, including supervision, support of learning objectives, and completion of student evaluations
- Create and maintain an individualized professional development plan through continuing education, self-study, and professional activities
- Adhere to discipline-specific State Practice Act and other regulatory agencies, and complete required state continuing education
- Uphold professional standards and ethical conduct consistent with discipline-specific codes of conduct
- Support performance improvement by following action plans and providing feedback through staff meetings, focus groups, or direct supervisor input
