Job Summary: Physical Therapist Outpatient Santa Monica, CA
Outpatient Physical Therapist (PT) opportunities are available in Santa Monica, California. The role evaluates assigned patients, develops and implements individualized treatment plans, and supports progress toward discharge in alignment with physician direction and established goals. Candidates are required to bring their license on the first day.
Job Details
- Schedule: Days shift, 8 hours per shift, with a minimum of 40 hours per week
- Required Qualifications: Degree in Physical Therapy from an accredited school; CPR Certification; outpatient experience; ability to maintain an active/applicable license for the state where work will be performed; 6 months to 1+ year of outpatient clinical experience
- Preferred Qualifications: Experience successfully completed an assignment with the organization previously
- Responsibilities:
- Completes comprehensive evaluations of assigned patients and uses clinical reasoning to develop treatment plans and goals focused on discharge within an established length of stay
- Completes timely, accurate medical record documentation, including documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry in approved formats in compliance with applicable guidelines and clinic policies
- Ensures care is provided under signed, valid physician orders and within insurance approval when indicated for clinical care
- Participates in student programs as assigned, including providing supervision, developing learning objectives, and completing student evaluations
- Creates and maintains an individualized professional development plan, identifying and initiating professional growth through continuing education, self-study, and professional activities
- Applies learned information to support organizational strategic objectives
- Adheres to the discipline-specific State Practice Act and other regulatory agencies, maintains licensure in good standing, and completes required continuing education
- Performs in an ethical manner in accordance with discipline-specific professional codes of conduct
- Supports performance improvement methodology by following action plans and offering feedback through staff meetings, focus groups, or direct supervisor communication
- Additional Information: Follows clinic guidelines outlining standards of personal appearance, attendance, and punctuality; demonstrates behaviors and attitudes consistent with the organizations mission and core values; upholds professional standards and compliance expectations, including confidentiality regarding patient and company information; maintains a patient- and employee-safety focus in alignment with established safety expectations and guidelines
