Description

Physical Therapist Outpatient (PT OP) Punta Gorda, FL
The organization is seeking an Outpatient Physical Therapist to evaluate assigned patients, develop and implement individualized treatment plans, and support progression toward discharge goals. This role includes timely, accurate medical record documentation, adherence to valid physician orders, and participation in professional development activities. Candidates are required to bring their license on the first day.

Job Details

  • Schedule: Days; 8 shift hours; minimum 40 hours per week
  • Required Qualifications: Degree in Physical Therapy from an accredited school; 1+ year of outpatient clinical experience; CPR certification; active/applicable license for the State where work will be performed
  • Preferred Qualifications: Prior successful completion of an assignment with the organization
  • Responsibilities:
    • Complete comprehensive evaluations of assigned patients and use clinical reasoning to develop appropriate treatment plans and goals
    • Provide care under signed, valid physician orders and within insurance approval, as indicated and necessary for clinical care
    • Maintain clear, concise, accurate, and timely medical record documentation, including documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry in accordance with clinic policies and procedures
    • Participate in student programs as assigned, providing supervision and supporting learning objectives and student evaluations
    • Create individualized professional development plans with clinical management and pursue professional growth through continuing education, self-study, and professional activities
    • Adhere to the discipline-specific State Practice Act and other regulatory agencies; maintain professional licensure in good standing and complete state-mandated continuing education requirements
    • Perform in an ethical manner in accordance with discipline-specific professional code of conduct
    • Support organizational performance improvement methodology by following action plans and offering feedback through appropriate team processes
