Job Summary
Occupational Therapist (OT) specializing in Skilled Nursing Facility (SNF) settings in Oklahoma. This role involves screening and evaluating patients with cognitive and Activities of Daily Living (ADL) physical functioning disorders, and developing appropriate care plans in accordance with regulatory standards. Provides rehabilitative, skilled, and medically necessary treatment interventions, utilizing standardized assessments and evidence-based practices to support clinical decision-making.

Job Details

• Schedule: 8-hour shifts, minimum of 40 hours per week.
• Required Qualifications: Active Occupational Therapist (OT) license, one (1) year of occupational therapy experience, including long-term care (LTC) or rehabilitation settings, and successful submission of a skills checklist within one year.
• Preferred Qualifications: Experience with elderly or rehabilitation populations preferred.
• Additional Information: Effectively communicates with patients, families, caregivers, and the healthcare team regarding patient progress and treatment plans. Documents all clinical activities in electronic health records, including treatment minutes and discharge summaries, adhering to all regulatory and company policies. Participates in care conferences, in-services, and ongoing training. Ensures supervision of occupational therapist assistants, rehabilitation aides, or students as required. Adheres to all NBCOT and state practice acts, codes of conduct, and ethics.
