Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Certifications in physical therapy, a valid state licensure, at least 1 year of experience in home health, and proficiency with electronic health record systems such as Epic.
• Preferred Qualifications: Experience working in outpatient or home health environments, and familiarity with OASIS documentation.
• Float: Float: Float rotation may be required.
• Additional Information: The position involves working within specified geographic zones, requiring adaptability to working in patients homes and in accordance with facility protocols.
