Job Details:

• Required Qualifications: Valid state licensure as a Physical Therapist; Certification in Basic Life Support (BLS); Degree in Physical Therapy from an accredited program; minimum of 3 years of experience in home health care; eligible to work in the United States.
• Preferred Qualifications: Experience with pediatric and adult patient populations; proficiency in electronic medical records.
• Additional Information: The role involves delivering skilled therapy interventions in patients' homes, collaborating with care teams, and adhering to all federal and state regulations. The position may require traveling within the specified service area to provide patient care.
