The role requires direct patient care, supervision of therapy assistants, and coordination of discharge planning, adhering to state-specific regulations.

Job Details

• Schedule: Days, 8 hours per day, minimum of 40 hours per week, starting on 08/03/2026.
• Required Qualifications: Current and unencumbered license to practice physical therapy specific to Georgia, with 0 or more years of experience. Successful completion of a physical therapy program approved by the American Physical Therapy Association or equivalent. Current CPR certification. Familiarity with general computer use, internet, email, and online HR and training platforms.
• Preferred Qualifications: 2-3 years of physical therapy experience preferred. Experience in home health settings is desired.
• Additional Information: The provider must have reliable transportation, a current drivers license, and agency-required liability insurance. The assignment involves traveling approximately 100 miles weekly and conducting 30 patient visits. The candidate will use Electronic Medical Records such as HomeCare HomeBase, Oasis, and Outlook. There will be on-call responsibilities approximately every 4th weekend. The position involves supervision of no more than 2 full-time equivalent Physical Therapy Assistants, providing on-site assessments, reevaluations, and treatment modifications at least once every 14 days or every tenth treatment day.
