Job Summary:
A Outpatient Physical Therapist position is available in Crown Point, Indiana. Job Details:

• Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Degree in Physical Therapy from an Accredited School; 1 or more years of outpatient clinical experience; CPR certification; active/applicable license for the state of Indiana; minimum of 0 years of experience.
• Schedule: Day shift, 8 hours per day, minimum 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an Accredited School; 1+ Year of Outpatient Clinical Experience; CPR Certification; active state license; minimum 0 years of experience.
• Preferred Qualifications: Has successfully completed an assignment with your agency previously (highly desired); has successfully completed an assignment with the organization previously (nice to have); not a recent graduate.
• Additional Information: The employee will perform comprehensive patient evaluations, develop treatment plans focused on discharge goals, complete accurate documentation in compliance with guidelines, participate in student training, and pursue ongoing professional development to meet organizational objectives. The role emphasizes adherence to professional standards, patient safety, and quality improvement initiatives.
