Job Details:
- Schedule: Day shifts; 8 hours per day; minimum of 40 hours per week.
- Required Qualifications: Valid active license in the state of Arizona, with at least 11 years of professional experience as a Physical Therapist in outpatient settings.
- Additional Information: Credentialing requires contract quality assurance; all equipment will be provided during the interview; dress code will be communicated on the first day. Parking will be provided during the first day.
