Job Summary
Occupational Therapist Home Health (OT HH) The organization is seeking a dedicated Occupational Therapist specializing in Home Health to provide patient-centered care within the North Napa Valley area. This role involves managing a caseload of geriatric patients, conducting daily visits, and ensuring compliance with clinical documentation standards.

Job Details

• Certifications, Education, and Experience Requirements: BLS certification, valid drivers license in hand, minimum of 1 year of relevant experience, proficiency in Homecare Homebase, and OASIS experience required for staff completing Start of Care for Home Health.
• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Responsibilities: Managing a caseload of approximately 5-7 patients daily, completing timely documentation, and maintaining communication standards.
• Additional Information: All travelers must have active transportation, submit Requested Time Off at application, and be flexible with territory. On-call rotation may be required. Modules are billable and completed during orientation. Former permanent employees must be separated for at least 6 months before returning as travelers.
