Description

Job Summary:
Occupational Therapist Home Health in Napa, California. This position involves providing home health services with a focus on geriatric patients, requiring recent experience in home health settings and proficiency with OASIS and HCHB systems. The role offers a flexible territory within up to a 60-mile radius from the designated hub.

Job Details:

  • Schedule: Monday through Friday, 8-hour shifts, with a minimum of 40 hours per week
  • Required Qualifications: Valid license in California, at least 1 year of relevant experience, recent home health experience including at least 6 visits daily with a minimum of 30 points weekly for productivity; BLS certification; active driver's license and personal transportation; experience with OASIS for SOC (Start of Care) in home health; proficiency in HCHB (Homecare Homebase)
  • Preferred Qualifications: None specified
  • Float: Float rotation may be required
  • Additional Information: Clinicians are expected to be flexible with territory coverage, ensure timely documentation and communication, and adhere to dress code of scrubs in any color (no jeans). No cell phone provision; iPads are provided. On-call rotation may be required and should be discussed during interview. All travelers must submit RTO requests at application. Orientation occurs on Monday.
  • Experience: Minimum of 1 year relevant experience, including home health and OASIS experience for SOC cases.
  • Patient Care: Serves geriatric patient population, managing about 5-7 visits per day.
