Job Summary:
The Physical Therapist Home Health (PT HH) position is based in Medford, Wisconsin. This role involves evaluating, treating, and planning patient care for adult and geriatric populations within a home health setting. The therapist will work under established standards of practice, documenting care using electronic medical records, and collaborating closely with patients to meet their rehabilitative needs.

Job Details:

• Location: Medford, Wisconsin
• Schedule: Days shift, Monday through Friday, 8:00 AM to 5:30 PM
• Guaranteed Hours: 40 hours per week
• Contract Length: 13 weeks with the possibility of extension
• Start Date: August 24, 2026
• Required Qualifications: Master's Degree in Physical Therapy, Wisconsin state licensure, Additional requirements include American Heart Association (AHA) or ARC Basic Life Support (BLS) certification, and a National Provider Identifier (NPI). Candidates must have prior home health experience, preferably working with adult/geriatric patients with orthopedic, neurological, and mobility diagnoses. At least one year of experience is preferred.
• Preferred Qualifications: Experience working with a diverse adult and geriatric population, resilience, strong work ethic, adaptability, and social skills.
• Additional Information:
  • Occasional travel to central and northern regions may be required, with mileage reimbursed at the government-approved rate.
  • The Saturday rotation involves working one Saturday every 6-8 weeks, with a weekday rotation off. No holidays are included.
  • During flu season, unvaccinated travelers are required to wear a mask at all times per facility policy.
  • Scrubs or business casual attire are acceptable.
  • The contract includes 36 hours of low census unbillable hours.
