Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week
• Required Qualifications:
  • Occupational Therapy license for Michigan
  • 2+ years of current home health experience
  • 2+ years of current occupational therapy experience
  • OASIS experience
  • Proof of auto insurance and a clean driving record
  • Active American Heart Association Basic Life Support (BLS) certification
  • Verified work history and skills relevant to occupational therapy and home health, current within the past year
  • Two supervisory reference forms completed within the last two years
  • Perm address must be included in the profile
  • All scheduling requests must be made at the time of submission; no requests can be accommodated after offer or interview

• Additional Information:
  • No holiday or on-call coverage required
  • Dress code: Olive green scrubs
  • Typical daily mileage: approximately 35-50 miles, paid at current Federal Rate for travel to and from the main office
  • No RTO will be accepted for this assignment; candidates residing within 50 miles of the facility will not be considered

  • Orientation: 8 hours, non-billable
