Job Summary:
The Physical Therapist Outpatient (PT OP) position is located in Avondale, Arizona. This role involves providing outpatient therapeutic services within a designated healthcare provider setting. The assignment requires adherence to clinical standards and documentation protocols, with an emphasis on timely licensure verification and compliance with facility-specific requirements.

Job Details:

• Schedule: Days shift, 8 hours per day, with a guaranteed 40 hours per week (32 hours during holiday weeks).
• Required Qualifications: Active Physical Therapy license (or eligible licensure status), at least 3 years of experience in outpatient physical therapy, location in Arizona or eligibility to obtain licensure, and a minimum of 0 years of experience.
• Additional Information: Candidates must start on or before 08/17/2026 to qualify for the $2,500 completion bonus. Candidates must provide proof of license application within 5 business days of booking and include license status in the submission email. This assignment is for a healthcare provider located at 1729 N Dysart Road, Avondale, AZ 85392. Equipment, parking, and dress code information will be provided during orientation and interview.
• Certifications & Credentialing: The clinician must complete additional documentation outside of the AMN Healthcare clearance packet prior to final clearance. South Carolina candidates require a permanent license; compact licenses are not accepted. Candidates with prior ATI employment within the past year are ineligible for rehire.
