Job Summary:
Physical Therapist Home Health position in the designated service area. This role involves providing outpatient therapy services to adult and geriatric patients within a home health setting. The therapist will assess patient condition, develop and implement treatment plans, and collaborate with an interdisciplinary team to promote patient recovery and well-being.

Job Details:

• Schedule: 8 hours per day, with a minimum of 40 hours per week, starting on 09/21/2026 and ending on 12/21/2026.
• Required Qualifications: Valid state license for Physical Therapy, BLS (Basic Life Support) certification, and a minimum of 2 years of experience in physical therapy, with at least 6 months of home health experience.
• Preferred Qualifications: Previous experience in home health care, strong assessment and critical thinking skills, excellent communication, and the ability to work collaboratively with patients and team members.
• Additional Information: Clinicians must demonstrate organizational skills, efficiency, flexibility, and strong assessment skills. Prior experience with adaptive devices, patient education, and working within electronic charting systems such as EPIC is preferred. The role requires collaboration with physical therapists and other healthcare providers to ensure comprehensive patient care throughout the home health setting.
