Job Summary
Pharmacist Inpatient (PHARM IP) Location: Not specified. This role involves managing medication processes, ensuring medication safety, and providing patient education within a healthcare facility. The pharmacist will perform responsibilities in accordance with state licensing regulations and institutional standards, supporting optimal medication therapy and safety for diverse patient populations, from neonates to adults.
Job Details
- Certification: RPH - Pharmacist License for the State of Texas, with a valid, unrestricted license to conduct clinical activities.
- Education Requirements: Bachelor's Degree in Pharmacy or Pharm.D. (Doctor of Pharmacy).
- Location Requirements: Not specified.
- Years of Experience: 0.0 years minimum.
- Schedule: 7:00am 7:30pm, including weekends.
- Criteria include ensuring medication doses are appropriate based on diagnosis, weight/body surface area, allergies, and drug interactions, validated by lack of medication incidents.
- Accurate entry of medication orders in the computer system, with verification ensured by absence of medication incidents.
- Ensuring medications received by patients are correct, with compliance demonstrated through documentation of no dispensing errors.
- Processing medication orders to meet established guidelines, validated by peer review or absence of complaints.
- Following aseptic technique and infection control practices during sterile product preparation, with annual re-certification scores of at least 90%.
- Active participation in patient education, discharge medication review, and supporting medication management processes.
- Ensuring efficient pharmacy operations and serving as a role model for technicians and other pharmacists.
- Maintaining investigational drug records as per departmental policy.
- Contributing to medication safety initiatives and supporting patient safety systems.
- Conducting discharge medication reviews and counseling to ensure patient understanding of therapies.
- Participates in study initiation, maintenance, and closure processes.
- All candidates are required to undergo respiratory fit testing in accordance with occupational health standards.
