Job Summary
Occupational Therapist SNF (Skilled Nursing Facility) Located in Texas, this position involves providing rehabilitative services to patients with cognitive and Activities of Daily Living (ADL) physical functioning disorders. The role includes screening, evaluating, and developing care plans based on regulatory and clinical standards to support patient recovery and independence.

Job Details

• Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: OT license, 1 year of OT experience (Level II preferred), and Texas licensure.
• Shift: Days, 8-hour shifts, minimum 40 hours per week.
• Provides rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and ADL physical functioning disorders based on regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practices to support clinical interventions.
• Administers procedures including Activities of Daily Living retraining, therapeutic activities, exercises, modalities, supportive and assistive device use, and more.
• Records treatment minutes and documents all regulatory and clinical information accurately in the electronic documentation system.
• Assesses patient progress and adjusts treatments accordingly, evaluating treatment outcomes for effectiveness.
• Offers consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Communicates effectively with team members regarding patient progress, barriers, and treatment plans.
• Supports supervision of occupational therapist assistants, rehabilitation aides, and students.
• Participates in meetings such as patient care conferences, utilization review, and other required trainings and conferences.
• Able to obtain necessary DME or supplies per regulatory and clinical requirements.
• Adheres to NBCOT / AOTA and State practice acts, ethics, and professional conduct during practice.
• Submission requirements: 1 year of OT experience; new grads with Level II experience are acceptable.
