Job Summary
Occupational Therapist (OT) Home Health Rhinelander. This position involves providing occupational therapy services to geriatric patients within the designated territory. The role requires delivering assessments and interventions to support patient recovery and independence in their home environment.

Job Details

• Specialty: Occupational Therapist (OT) Home Health
• Location: Rhinelander (between Rhinelander / Arbor View) with a territory of up to a 60-mile radius from the hub/office
• Shift Time: Days, 8 hours per shift, with a minimum of 40 hours per week
• Certifications: BLS (Basic Life Support) required
• Years of Experience: 1 year minimum, recent experience preferred.
• Clinical Experience: Previous charge, charting system experience required, including with home health IS systems such as HCHB (Homecare Homebase).
• Knowledge of patient care across acute, outpatient, and community settings preferred.
• Additional Requirements: License in hand, active drivers license, personal transportation, timely documentation and communication, and availability for on-call rotation (every 3rd to 4th weekend).
• Responsibilities include providing therapy interventions, documentation, and participating in interdisciplinary care.
• Orientation begins on Monday, with specific modules to be completed during orientation.
• On-call rotation may be required; details discussed during interview.
• RTO (Requested Time Off) must be submitted at the time of application.
• Must be flexible with territory and coverage of geriatric cases, with up-to-date OASIS experience for staff completing Start of Care (SOC) assessments.
