Job Summary
Occupational Therapist SNF in Texas. This healthcare organization is seeking a dedicated Occupational Therapist to provide skilled services within a Skilled Nursing Facility environment. The role involves evaluating patients with cognitive and activities of daily living (ADL) physical functioning disorders, developing individualized care plans, and utilizing evidence-based practices to improve patient outcomes.

Job Details

• Position Description: Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders. Develops appropriate plans of care, utilizing standardized assessments and evidence-based practices. Provides rehabilitative, skilled, and medically necessary treatment interventions, including ADL retraining, therapeutic activities, exercises, modalities, and supportive device use.
• Required Qualifications: Occupational Therapist (OT) license, Texas licensure, Bachelors degree in Occupational Therapy, 1 year of OT experience, ability to work day shifts (8 hours), minimum of 40 hours per week, starting on 10/12/2026, ending on 01/11/2027. New graduates with Level II experience are acceptable.
• Additional Information: Administers procedures as part of the rehabilitation plan, assesses patient progress, and adjusts treatments accordingly. Evaluates treatment outcomes, offers consultation and counseling to patients and families, documents all services and patient progress, participates in team meetings, and supports supervision of therapy assistants and students. Adheres to professional practice acts, codes of conduct, and ethics.
