Job Details

Certification: Active Pennsylvania license required
Experience: 2+ years in home health therapy, with start of care and revisit experience
Location Requirements: Servicing zip codes 19040, 19044, 18929, 18974, 18976, 19002, 18954, 18966, 18901
Schedule: Days shift, 8-hour shifts, minimum of 40 hours per week
Position Type: Part-time with a start date of 10/21/2026 and end date of 01/16/2027
Responsibilities include delivering patient-centered physical therapy, documenting patient progress, and collaborating with care teams to meet patient goals.
Submission Requirements: Active PA license required to submit
