Job Summary
Home Health Physical Therapist (PT HH) Duration: October 12, 2026, to January 11, 2027. This position involves providing skilled physical therapy services in a home health setting. The role requires delivering patient care based on individualized plans and working within a multidisciplinary team to promote patient recovery and mobility.
Job Details
- Schedule: Days, Monday through Friday, 8:00 AM to 5:00 PM, with a minimum of 40 hours per week.
- Required Qualifications: CA License, BLS (AHA or ARC) certification, and a minimum of 2 years of experience.
- Responsibilities include delivering home health physical therapy, adhering to clinical protocols, and documenting patient progress accurately using Cerner charting system.
- Travel: Mileage is billable according to GSA rates; approximately 25% travel is expected during assignment.
- Additional Information: The assignment may be canceled up to 4 shifts within a 13-week period or 3 shifts for shorter periods, with provisions to pay for full hours in lieu of cancellations.
