Description
description:
Provides excellent patient care by assisting in the collaboration, development, implementation, revision and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items. Coordinates a continuum of care for a defined population of patients from pre-admission through post discharge. Assures that patients and families proceed efficiently through the course of hospitalization.
requirement:
Day Shift, Sun-Thurs or Tues-Sat, 8 to 4:30 or 8:30 to 5. No On Call. need 5 yrs case manager experience. need RN licensure in CA. Need to be able to manager high volume, ratio 1:25, if short staff could be more. Ideal candidate that has worked for HCA hospital. per CA experience (maybe so cal). be able to use Cerner. experience with Interquall is a plus. possible extension, need to be able to start right away. if a good fit may be able to transition to FTE. If local would like to do in person interview. If out of area they will do TEAMS interview.
Education and Work Experience:Bachelor's Degree in Nursing (BSN): PreferredExperience in a care management role: PreferredLicenses/Certifications:Registered Nurse (RN) licensure in the state of practice: RequiredCase management certification: PreferredDepartment Specific License/Certifications:Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required
