Description
Advanced Care Partners is seeking a Licensed Practical Nurse (LPN) to provide one-on-one care for medically fragile children and adults in home and school settings. This role offers the opportunity to deliver high-quality, individualized care in a more focused and meaningful environment.
Compensation & Benefits
Base Pay: $35/hour
Opportunity to earn up to $43/hour with applicable differentials:
Trach: +$2/hour
Vent: +$3/hour
Nights (7pm–7am): +$2/hour
Weekends: +$3/hour
Holidays: +$3/hour
Weekly pay

Key Responsibilities
Deliver skilled nursing care, including G-tubes, tracheostomies, ventilators, and central lines
Monitor patient condition and respond appropriately to changes
Educate patients and families on care plans and procedures
Collaborate with interdisciplinary team members to ensure optimal outcomes
QualificationsQualifications
Active LPN license in Georgia or Nurse Licensure Compact
A minimum of one year of LPN experience required.
Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or an equivalent accredited organization required.
Valid PPD skin test
Physical Requirements of the Role
Ability to stand or sit for extended periods (up to 12 hours)
Ability to bend, twist, kneel, and assist with patient mobility
Ability to lift, push, or pull up to 50 pounds or more
Manual dexterity for clinical tasks
