PT (Physical Therapist)

Job Description:

The Physical Therapist (PT) will provide therapy services in a home-based care setting, assigned to a specific geographical region, working primarily Monday through Friday. Candidates must have experience with Oasis and a minimum of one year of experience in PT/OT/Sp. Home setting experience is preferred but not required. The candidate must possess a valid BLS Certification and should follow the business attire dress code with a white lab coat.

Responsibilities:

Deliver physical therapy services to assigned patients.
Complete government-mandated forms as required (Oasis).
Coordinate with Rehab Aides/OTA/PTA for patient care.
Work flexible hours, with the majority scheduled Monday through Friday.
Participate in floor-specific orientation upon start.

Requirements:

BLS Certification
Experience with Oasis documentation
Minimum one year of PT/OT/Sp experience
Ability to work every 3rd weekend as needed
