Home Health Physical Therapist

Job Description:
Provide physical therapy services to patients in a home health setting. Responsibilities include assessing patient conditions, developing treatment plans, and documenting progress using Meditech and RehabConnect systems. Work as part of a dedicated rehab facility team with no floating required.
Responsibilities:
- Conduct patient assessments and develop individualized therapy plans.
- Utilize equipment such as Sara stedy, Sara plus, hoyer lifts, and sliding boards.
Requirements:
- Experience in an inpatient rehab hospital preferred; proficiency in safe patient handling techniques.
- Current licensure to practice in North Carolina; familiarity with protocols to prevent complications.
