Description
Job Description
We are seeking a dedicated Home Health Physical Therapist to join our team. This role involves providing high-quality physical therapy services to patients in their homes to enhance their quality of life and promote their recovery.
Key Responsibilities
- Assess and evaluate patients' physical conditions and limitations.
- Develop and implement personalized treatment plans.
- Provide therapeutic interventions to improve mobility, strength, and endurance.
- Educate patients and their families on treatment options and care strategies.
- Document patient progress and modify treatment plans as necessary.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
Qualifications
- Must possess a valid California Physical Therapist license.
- Certification in Basic Life Support (BLS) from AHA or ARC is required.
- Minimum of 2 years of experience in physical therapy.
- Strong interpersonal and communication skills.
- Ability to work independently and manage time effectively.
Work Schedule
This position requires availability for days, Monday through Friday, from 8 AM to 5 PM.
Travel Requirements
Position requires approximately 25% travel to patient locations within the assigned area. Mileage will be compensated at the standard GSA rate.
