Description
Job Summary
We are seeking a dedicated and compassionate Home Health Physical Therapist to provide high-quality therapy services to patients in their homes across the region. This position requires strong clinical skills and the ability to work independently while ensuring optimal patient care.
Shift Information
- Shift: Days, Monday-Friday
- Hours: 8:00 AM - 5:00 PM
Key Responsibilities
- Conduct evaluations and assessments of patients.
- Develop and implement individualized treatment plans.
- Provide skilled physical therapy services in patients' homes.
- Monitor patient progress and adjust treatment plans as needed.
- Collaborate with healthcare team members to ensure comprehensive patient care.
- Document patient care and maintain accurate records.
Qualifications
- Valid California Physical Therapist License.
- Minimum of 2 years of experience in physical therapy, with home health experience preferred.
- Current Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC).
- Strong interpersonal and communication skills.
- Ability to work independently and manage time effectively.
- Willingness to travel; approximately 25% travel is required for this position.
Additional Information
Mileage reimbursement is provided at the General Services Administration (GSA) rate.
