Job Description

• Outpatient Physical Therapy with a preference for therapists willing to take occasional hospital shifts.
• Shifts: 4 shifts of 10 hours scheduled on weekdays, starting between 8:00 - 8:30 AM.
• Shared housing provided at no cost.

Requirements

Licenses and Certifications

• Massachusetts PT License: Valid and required.
• BLS Certification: Must be current and from AHA, Red Cross, CHSF, or Military.

Health and Safety Credentials

• COVID-19 Vaccine: Completed primary series or up-to-date vaccination required; no exemptions or waivers accepted.
• MMR Vaccination: Documentation of two measles, two mumps, and one rubella vaccine or proof of immunity required.
• Hepatitis B: Record of three vaccines or proof of immunity preferred.
• Influenza Vaccine: Required during flu season (Sept 15 - April 30); no exemptions or waivers accepted.
• TB Test: Documentation of a TB skin test or negative QuantiFERON TB test required within 3 months of start date.
• Health Statement: Signed by MD/DO/NP/PA within 365 days of start date.
• Tdap Vaccine: Preferred if within 10 years.

Background Checks

• Criminal Background Check: Required within 45 days prior to start date.
• Massachusetts CORI Check: Required within 30 days prior to start date.
• National Sex Offender Check: Required within 30 days prior to start date.
• Exclusion Checks: Required for OIG, GSA, and MassHealth & NH Medicaid exclusion lists.

Documentation

• Resume: Must include past 7 years of work experience with city & state.
• References: Two recent references required, with at least one from a supervisor or above.
• Skills Checklist: Required for submission.
• Confirmation of Assignment/POI Form: Required upon acceptance of assignment.

Additional Information

• All health and safety credentials must be completed prior to onboarding.
• Documentation must be uploaded as specified, and all requirements must be met without exceptions for submission.
