Job Description

• Responsibilities:

  • Conduct home visits for therapy patients.
  • Participate in periodic office meetings.
  • Must have reliable transportation, a current driver's license, and agency-required liability insurance.

• Patient Load:

  • Average of 5-6 patients per day.
  • Points system for productivity: 30 points per week, with each type of visit and activity holding a different point value.

• Types of Cases:

  • Start of Care and Follow-Ups.

Requirements

• Experience:

  • Minimum of 1 full year of Adult Home Health experience.
  • Recent Home Health experience is mandatory; first-timers are not accepted.

• Certifications:

  • CPR and BLS required.

• On-Call Requirements:

  • Varies per job/location; to be discussed during the interview.

• Travel:

  • Must be willing to travel up to 75 miles from the home clinic.
  • Mileage reimbursed at the IRS standard rate.

• Local Candidates:

  • Locals accepted at the same rate.

• Licensing:

  • Must have a valid license in hand; pending licenses are not accepted unless they are expected to be obtained shortly.

Schedule

• Typical Hours: Monday to Friday, 8 AM to 5 PM.
• Weekend Requirements: Varies by location and profession; to be discussed during the interview.

Additional Information

• EMR Systems: HomeCare HomeBase, Oasis, Outlook.
• Productivity Expectations: 30 points per week.
• On-Call: 1 weekend per month and 1 summer holiday.
