Job Opportunity: Cath Lab Technician (Travel)

Certifications & Licenses

• BLS (American Heart Association certification): Required for all direct patient care personnel. Must provide scanned copy of both front and back of card with expiration date.
• License/Certificate Verification: Must be dated within 30 days of start. Attach scanned online verification with machine-generated date.
• Fit Test: Required.
• Annual TB Questionnaire: Required.
• Trinity 9 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include cocaine, amphetamines, opiates, and other specified substances (NO THC).
• COVID-19 Vaccine: Proof of vaccine, booster (Sept 2024), declination, or exemption required (declination not accepted for CA or IL).
• MMR Vaccine: Proof of 2 vaccinations or primary source IgG titer with lab printout.
• Hepatitis B: Must have first vaccination prior to start; subsequent doses as per series. Titers not accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption (declination not accepted for CA or IL). Flu season dates: 10/1/2024 - 4/30/2025.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required.
• Varicella: Proof of 1 vaccination or IgG titer with lab printout.
• Tdap Vaccine: Current, non-expired required. Declinations accepted if no pediatric interaction.

Health & Safety

• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Trinity Specific COVID Questionnaire: Required.
