Job Opportunity: Physical Therapist - Home Health (Travel/Contract)

Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and periodic office meetings.
- Average patient load: 5-8 patients per day.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Must be willing to travel up to 75 miles from the home clinic.
- Patient visits include both patient homes and care facilities.
- Schedule typically Monday to Friday, 8 AM to 5 PM, with weekend coverage rotation.
- Use of electronic medical records (EMR) systems: HomeCare HomeBase (HCHB), OASIS, and Outlook.
- Dress code: Scrubs.
Requirements
- Minimum 1 full year of recent Adult Home Health or Hospice experience (depending on specialty).
- First-time travelers not accepted; must have recent specialty experience.
- CPR and BLS certification required.
- Must have a current, valid license in hand (no pending licenses accepted).
- Reliable transportation, current driver's license, and agency-required liability insurance required.
- Proficiency with HomeCare HomeBase (HCHB) within the last 6 months; retraining required if last used over 6 months ago.
- RNs, PTs, and OTs must be proficient in OASIS documentation.
- Weekend and on-call requirements vary; discuss during interview.
Productivity Expectations
- Weekly productivity units: approximately 25-30.
- Daily travel: 30-40 miles.
- Compensation includes mileage reimbursement at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).
Compliance and Documentation
Candidates must complete and maintain the following prior to and during assignment:
- Valid Driver’s License/State ID.
- Background checks including 7-year history, sex offender search, and state-specific screenings.
- Fingerprinting as required by state and facility.
- Health documents including:
- Physical exam within 12 months.
- Negative TB test or chest X-ray within 12 months (or facility-specific TB questionnaire if unable to test).
- Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
- Drug screening (10-panel).
- Seasonal flu vaccination or declination.
- COVID-19 vaccination or declination.
- Completion of facility-specific compliance training and policy acknowledgments, including but not limited to:
- HIPAA and Privacy Policy.
- Code of Conduct.
- Confidentiality agreements.
- Home Health and Hospice Core Policy acknowledgments.
- COVID-19 Policy and Attestation.
- Completion of OASIS E-Course if not recently proficient with HCHB.
Additional Notes
- Local candidates are accepted at the same rate.
- Contract may potentially lead to permanent hire.
- Must be willing to adhere to all agency and facility compliance requirements, including annual renewals if working consecutively for more than one year.
- Some positions may have alternative shift schedules (e.g., 4x10s Friday-Monday), but most are standard 5x8 Monday-Friday.
