Position Details

• Specialty: Therapy/Rehabilitation (Physical Therapy)

• Schedule: Monday to Friday, 8 AM to 5 PM, plus weekend call every 3 weeks
• Patient Visits: 5-6 patients per day, both in patients' homes and care facilities

• Mileage Reimbursement: IRS standard rate, compensated from patient home to patient home (commute to/from first and last patient not billable)
• Schedule Variations: Some positions may have Baylor Shift (4x10s, Friday-Monday), most are standard 5x8s M-F

Requirements

• Experience: Minimum 1 full year of recent Adult Home Health experience (no first-timers)
• Licensure: Must have current license in hand (pending licenses accepted only if a few days from issuance)
• Certifications: CPR and BLS (AHA, National CPR Foundation, or ARC accepted)
• Transportation: Reliable transportation and current driver's license required
• Insurance: Agency-required liability insurance required
• Willingness to Travel: Up to 75 miles from home clinic
• Background Checks & Compliance: Must pass standard background checks, drug screening, and meet all facility-specific compliance and documentation requirements

• Reason for Opening: One full-time PT moving to PRN status, creating workload need
• Contract End: Potential for hire if a good fit for the team
• Documentation & Compliance:

  • Drivers License/State ID required
  • Various state-specific releases and background checks depending on placement
  • Facility-specific documents including confidentiality forms, compliance training, HIPAA, privacy policies, and COVID-19 policies must be reviewed and signed prior to start
  • Health documents required include physical exam (within 12 months), negative TB or chest X-ray, immunization proof (MMR, Hep B, Varicella), drug testing, and seasonal flu vaccination or declination
  • Fingerprinting required as per state regulations
  • References required (two clinical references minimum)
