Job Opportunity: Physical Therapist - Home Health (Travel/Contract)
Location: O'Fallon, Illinois

Job Description
- Provide physical therapy services in patients' homes and some assisted living facilities (ALFs).
- Average caseload: 5-6 patients per day.
- Types of visits: Start of Care and Follow-Ups.
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM (some locations may vary).
- Travel: Must be willing to travel up to 75 miles from the home clinic; daily travel approximately 50-100 miles.
- Mileage reimbursed at IRS standard rate (patient-to-patient travel compensated; commute to/from first and last patient is not billable).
- Productivity expectations: 30 points per week (points assigned based on visit type and activities).
- On-call requirement: Approximately 1 weekend per month and 1 summer holiday (varies by location).
- Guaranteed hours: No, but options to float to nearby clinics if patient census is low.
Requirements
- Minimum 1 full year of recent adult home health experience (first-timers not accepted).
- Current, valid Physical Therapy license for Illinois (license must be in hand; pending licenses not accepted unless imminent).
- CPR and BLS certification required.
- Reliable transportation and current driver's license.
- Agency-required liability insurance.
- Must be willing to attend periodic office meetings.
- Ability to use EMR systems: HomeCare HomeBase, Oasis, Outlook.
- Local candidates accepted at the same rate.
Compliance and Documentation
Candidates must complete and maintain the following prior to and during assignment:
- Valid Driver’s License/State ID.
- CPR/BLS certification (AHA, National CPR Foundation, or ARC accepted).
- Personal or auto insurance policy (must be current and under candidate’s name or list candidate as a driver).
- Background checks including 7-year history, sex offender registry, and state-specific screenings.
- Drug screening (10-panel).
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months.
- Immunizations or proof of immunity for Measles, Mumps, Rubella, Hepatitis B, Varicella.
- COVID-19 vaccination or facility declination.
- Seasonal flu vaccination or declination.
- Completion of facility-specific compliance training, confidentiality agreements, HIPAA and privacy policies, and home health scenarios.
- Various state-specific forms and attestations depending on placement location.
Additional Notes
- Candidates must sign new supplier packets at the start of each assignment and annually if working more than one consecutive year.
- Fingerprinting requirements vary by state and must be completed prior to start where applicable.
- References: Two clinical references required; some states require two professional references.
- Candidates should be prepared for possible weekend and holiday on-call duties.
- Contract is potentially renewable or may lead to permanent hire depending on need.
