Position Details

• Schedule: Generally Monday-Friday, 8 AM - 5 PM; some locations may have different shift patterns (e.g., 4x10s Friday-Monday)
• Weekend Rotation: Typically none, varies by location
• On-Call: None or varies by location; discuss during interview
• Territory Coverage: Must be willing to travel up to 75 miles from home clinic; approx. 650 miles traveled weekly
• Local Candidates: Accepted at the same rate
• EMR System: Pointcare (HCHB)
• Patient Visits: Contractor assigned 5-10 patients/day, equating to roughly 6-8 points per day (30 points expected per week)
• Guaranteed Hours: No; if patient census is low, floating to nearby clinics may be arranged

Requirements

• Experience: Minimum 1 full year of recent Adult Home Health experience required
• First-Timers: Not accepted; must have recent home health experience
• Licensure: Must have license in hand (pending licenses accepted only if processing takes a few days)
• Certifications: Current CPR and BLS required (AHA, National CPR Foundation, or ARC accepted)
• Transportation: Reliable transportation and current driver's license required
• Insurance: Agency-required liability insurance and personal/auto insurance under candidate’s name or candidate listed as a driver on the policy
• Background Checks & Compliance: Must pass standard background checks, drug screening, and meet all state-specific requirements
• Health Requirements:

  • Physical exam within 12 months prior to start
  • Negative TB test or chest X-ray within 12 months, updated annually
  • Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella as per Aya standards
  • COVID-19 vaccination or facility declination required
  • Seasonal flu vaccination or declination documentation required

• Reporting Location: 1050 Bailey Drive, Demopolis, AL 36732

Additional Notes

• Candidates must review and sign various compliance, confidentiality, and policy documents prior to start and annually if working consecutively for more than one year.
• Mileage is reimbursed at the IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).
• Candidates should be prepared for thorough credentialing and onboarding processes including state-specific background checks, fingerprinting (where applicable), and other regulatory requirements.
• Productivity expectations are approximately 30 points per week, with points assigned based on visit types and activities such as documentation and travel.

This position is ideal for experienced physical therapists with a background in adult home health therapy who are comfortable with travel and working independently in a home health setting.
