Job Opportunity: Outpatient Physical Therapist

Location: Duncan, Oklahoma
Facility: Duncan Regional Hospital (Qualivis Version 2 system)
Employment Type: Travel/Contract

Job Details

• Specialty: Therapy/Rehabilitation
• ASAP start date
• Schedule: 4 days a week, 10 hours per day (4x10)
• Experience: Minimum 1 year required
• First-time travelers accepted
• No call or weekend shifts
• May float to acute care as needed
• Blocked scheduling accepted
• Must have an Oklahoma physical therapy license (pending OK license candidates considered)
• Required certifications: BLS and CPR
• Required to submit Return to Office (RTO) documentation at time of submission

Clinical Experience Requirements

• Outpatient rehab experience with:

  • Musculoskeletal diagnoses
  • Post-operative orthopedic diagnoses
  • Debility
  • Back pain
  • Neurological conditions

Required Certifications & Verifications

• BLS (Basic Life Support) - ARC, AHA, or US Department of Defense approved
• CPR certification
• State License verification (Primary Source Verification required)
• Background check: 7-year criminal history, updated annually
• Drug screening: 9 or 12 panel drug screen within required timeframe
• Immunizations and health documents including:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Negative TB test or chest X-ray (within 1 year)
  • Measles, Mumps, Rubella (2 vaccines or positive titer)
  • Hepatitis B (titer, vaccine series, or declination)
  • Varicella (2 vaccines, titer, or history)
  • Seasonal flu vaccination or approved exemption during flu season (Oct 1 - Mar 31)
• Physical exam within last 12 months (not required annually)
• Fit mask test (N95 or equivalent) within last year; on-site fit testing available if needed

Facility-Specific Requirements

• Completion of facility-specific forms prior to start, including:

  • Confidentiality and Non-Disclosure Agreement
  • Employee Health Survey
  • New Team Member Record Form (time sensitive)
• Flu vaccine exemption forms require physician signature (no electronic signatures accepted)
• Management of Assaultive Behavior (MAB) training accepted in lieu of CPI training (must be updated every 2 years)

Additional Competencies & Documentation

• Medication/Pharmacology competency exam as applicable
• Color blindness testing required for all clinical candidates
• HIPAA training and acknowledgment required
• References:

  • 2 professional references required for travel and allied positions
  • References must be within 2 years of start date
