Job Opportunity: Physical Therapist (PT) - Travel Assignment

Job Details

• Requires at least 1 year of hospital experience.
• Weekend shifts: 6 weekend days per 13-week assignment.
• No on-call duties.
• Holidays rotate with staff.
• May need to float onsite within skill set or to other sites within 40 miles if needed.
• Basic Life Support (BLS) and ARDMS certifications required.

Required Credentials & Compliance

Certifications & Exams

• Ascension Specialty Exam: Must be completed prior to first assignment and updated every 3 years. Minimum 80% passing score required. Only 2 attempts allowed.
• Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.

Health & Immunizations

• MMR: Documented proof of two MMR vaccines or positive IGG titer. No declinations accepted. If negative titer, two doses of MMR vaccine four weeks apart required.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Recommended during flu season (Oct 1 - Mar 31). Exemptions accepted but mask required at all times.
• Varicella: Proof of two vaccines, serological immunity, or verified history. No declinations accepted.
• Tdap: Valid within 10 years. No declinations accepted.
• Positive PPD/Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest X-ray within one year showing no infection, and completed TB questionnaire.
• 2 Step PPD OR IGRA: Required within specified timeframes prior to start.
• Fit Test: Required for all travelers, preferred before start but can be completed onsite. Annual update required.

Personal Health & Background

• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
• 12 Panel Drug Screen: Must be completed within 30 days prior to start and updated annually. Includes testing for 12 specified substances.
• Driver's License/State ID: Must be current and valid, matching address requirements for compact licenses.

Professional Documentation

• Ascension Profile (Resume)
• Two Professional References: At least two qualifying references completed prior to first assignment.
• Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender, and other checks. Must be completed within 30 days of first assignment and updated annually.
• Education Verification: Verification of education relevant to clinical work within 90 days of start.
• Employment Verification: Third-party verification for the past seven years, updated annually.
• Ascension Confirmation of Assignment: Provided upon initial assignment and updated with each extension.

Additional Notes

• A forward-facing headshot with a white or blue background is required for badge photo.
• Legal documentation of name change must be provided if applicable.
• Post-assignment evaluation may be requested.
